The fine print:
Insurance Billing and Patient Financial Responsibility
We will submit claims to your insurance carrier as a courtesy.
However, verification of benefits or pre-authorizations (if obtained) does not guarantee payment. Coverage, eligibility, and payment amounts are determined solely by your insurance plan.
You are ultimately responsible for all charges not covered or paid by your insurance, including but not limited to deductibles, co-payments, co-insurance, non-covered services, denied claims, or any remaining balance after insurance processing.
Payment for any patient-responsible portion is due at the time of service or upon receipt of your statement, unless other arrangements have been made.
Other Payments
The fine print:
Returned checks (for insufficient funds or any other reason) will incur a $50 processing fee.
If a credit card payment is declined, returned, charged back, or reversed, you will be responsible for any associated fees or costs incurred by our practice, and we reserve the right to pursue collection of the original amount plus fees.
All returned or disputed payments may result in your account being placed on hold or referred to collections.

Checks - We will do an ACH transfer on a check
Cash - we do not keep cash in the office, so if you want to pay in cash, you may. But we cannot make change.
